Welcome! We are thrilled that you have considered being matched with a virtual mentor. If you haven’t please take a second to read through the program requirements.
OUR MENTOR PROGRAM REQUIREMENTS INCLUDE:
Meet with your mentor at least once a month (virtual or in-person) from August - May
Develop jointly written goals (i.e.- documented and measurable) and sign the mentor program agreement with established goals.
Provide formal feedback through the Diversify Dietetics' evaluation process.

We use the mentor program questionnaires to learn more about you and to help us match you with the best possible mentor. Complete the questionnaire below and we will be in touch soon. You will receive your Mentor Program match & next steps by August 17th!
