This full-time, permanent position is located in Nutrition and Food Services (NFS), MEDVAMC. The Clinical Dietitian - Home Based Primary Care (HBPC) is recognized as the medical nutrition expert on the multi-disciplinary treatment team which includes physicians, nurses, and other allied health care professionals. This position provides support to the Home Based Primary Care Program.

Duties include, but are not limited to:

A. Nutritional Care:

  1. Conducts a comprehensive nutrition assessment for patients at nutritional risk.

  2. Develops and implement an individualized plan for medical nutrition therapy in accordance with the patient's medical program goals and objectives.

  3. Calculates nutritional requirements (calories, protein, carbohydrates, lipids, and fluid) and necessary diet restrictions or food/drug interactions.

B. Nutrition Education:

  1. Assesses educational needs and provides nutrition counseling for individuals and groups, incorporating a learning approach that considers the cultural, religious, educational, and medical needs of the patient. Includes information regarding budgeting, menu planning, food purchasing and preparation, food storage and sanitation.

  2. Evaluates achievement of learning objectives by the patient and/or family. Adapts nutrition education materials and counseling techniques to client's level of understanding, which frequently involves simplifying complex dietary restrictions.

C. Quality Improvement/Research:

  1. Supports Performance Improvement activities by participating in the development of nutritional care indicators, evaluation criteria, collecting continuous quality improvement data, monitoring outcomes and initiating corrective actions.

  2. Participates in planning and developing nutrition-related policies, procedures, and goals.

D. Participation as a Member of the Health Care Team:

  1. Serves as the nutrition expert on the HBPC interdisciplinary treatment team which includes physicians, nurses, and other allied health professionals.

E. Professional Development:

  1. Maintains dietetic registration and continuing education requirements.

  2. Develops and implements an individualized plan for professional growth and development including participation in professional organizations and activities, workshops, seminars, and staff development programs.

Customer Service:

  1. Meets the needs of customers while supporting VA missions.

  2. Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.

Work Schedule: 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, Alternative tours may be necessary to support patient needs
